Get Started<\/b><\/h2>\n\n\n

<\/p>\n<\/div>[vc_column_text el_class=”a-link”]You don\u2019t need a professional referral to receive services. Simply visit our Roosevelt or Vernal offices during regular business hours (Monday \u2013 Friday, 8 AM to 5 PM) to schedule an appointment.\u00a0 Our staff will provide you with intake paperwork to fill out. These must be completed before we can schedule your first appointment. If you are experiencing a mental health emergency, please request to speak with a crisis clinician.<\/span><\/p>\n

During the initial appointment a clinician will complete a thorough assessment. Our clinical team will then review it and recommend the appropriate services.\u00a0 Our support staff will contact you with treatment recommendations if applicable and the assigned counselor. If you have not heard from our support staff, please contact our office.\u00a0<\/span><\/p>\n

The links to paperwork are provided below. These can be printed or filled out using an Adobe PDF program.\u00a0 Once complete, email to the address listed on the form or deliver it to the office.\u00a0 After paperwork is received an appointment can be scheduled.<\/span><\/p>\n
