You don’t need a professional referral to receive services. Simply visit our Roosevelt or Vernal offices during regular business hours (Monday – Friday, 8 AM to 5 PM) to schedule an appointment. Our staff will provide you with intake paperwork to fill out. These must be completed before we can schedule your first appointment. If you are experiencing a mental health emergency, please request to speak with a crisis clinician.
During the initial appointment a clinician will complete a thorough assessment. Our clinical team will then review it and recommend the appropriate services. Our support staff will contact you with treatment recommendations if applicable and the assigned counselor. If you have not heard from our support staff, please contact our office.
